Should I Exercise With Back Pain?

When your back hurts, exercise might be the last thing you feel like doing.
Many people worry that exercise will make their pain worse, cause more damage, or slow down their recovery.
However, for many people with back pain, the right type and amount of exercise can actually be an important part of recovery.
The key is understanding how to exercise safely and knowing when to modify your activities.
Is exercise safe with back pain?
In most cases, yes.
Your spine is strong and designed to move. While back pain can feel alarming, avoiding movement completely can sometimes lead to reduced strength, stiffness, and less confidence with everyday activities.
Exercise can help by:
  • Improving strength and endurance
  • Increasing confidence with movement
  • Supporting your return to work, sport, and hobbies
  • Helping your body tolerate everyday activities
The best exercise programme is one that is appropriate for your current symptoms and your goals.
Should I rest if I have back pain?
When back pain is severe, a short period of reducing your activity can be helpful.
However, prolonged rest is usually not the best approach.
Staying gently active can help:
  • Prevent stiffness
  • Maintain movement confidence
  • Support recovery
This does not mean you should push through severe pain. Instead, the aim is to find the right level of activity and gradually build from there.
What exercises are good for back pain?
There is no single “best” exercise for everyone with back pain.
The most effective exercise programme depends on:
  • Your symptoms
  • Your current abilities
  • Your lifestyle
  • Your goals
A programme may include:
  • Strength exercises
  • Mobility exercises
  • Walking or cardiovascular exercise
  • Gradual return to lifting, running, or sport
The goal is not just to reduce pain — it is to help you return to the activities that are important to you.
Can I go to the gym with back pain?
For many people, yes.
In fact, continuing appropriate strength training can be a valuable part of managing back pain.
You may need to temporarily adjust:
  • The amount of weight you lift
  • The number of repetitions
  • Certain exercises that aggravate your symptoms
A good rehabilitation plan helps you gradually build back up rather than avoiding exercise forever.
What if exercise makes my back hurt?
Some discomfort during exercise does not always mean you are causing harm.
It is normal for your body to need time to adapt to new movements or increased loads.
However, it is important to pay attention to:
  • Whether symptoms are improving over time
  • Whether pain settles after activity
  • Whether your function is improving
A physiotherapist can help you understand what is a normal response and what may need modifying.
How can physiotherapy help me return to exercise?
A physiotherapist can help you:
  • Understand your back pain
  • Identify factors contributing to your symptoms
  • Develop a personalised exercise plan
  • Progress your activities safely
  • Return to the gym, sport, or hobbies you enjoy
Rather than avoiding movement, the goal is to rebuild your body's capacity and confidence.
When should I get help?
Consider seeing a physiotherapist if:
  • Back pain is stopping you from exercising
  • You are worried about making your injury worse
  • You are unsure what exercises are safe
  • Your symptoms are not improving
