Hip Pain: In another case, a runner started developing outer hip pain as she increased her pace. The outer hip muscles stabilise the pelvis during single-leg loading, which is precisely what happens during running. When the pace increased, the demand on those muscles increased with it. Why Rest Alone Doesn't Fix It

The urge to return to full training the moment pain settles is completely understandable, but it's also one of the main reasons people end up injured again. Pain Going Away Is Not the Same as Being Ready

Calf strains are one of the most common running injuries, and they can range from a persistent tightness to something that leaves you barely able to walk.

Here's a real case from our sports physio clinic in Palmerston North, showing exactly what happened to one runner, how we assessed her, and how we got her back running safely.

How the Injury Happened
Our patient was a relatively new runner who had been gradually building up her training. She had also been adding skipping into her routine, which, like running, places significant load through the calf. Over several weeks, her calf had been getting progressively tighter, but she kept going. Then one day during a run, it gave way. It wasn't the sharp pop you sometimes hear associated with a major muscle tear; it was a more severe version of the tightness she had been feeling, and it stopped her immediately. From that point on, she struggled to walk. She experienced constant calf pain, with occasional shooting pains running down into her leg.

What Is a Calf Strain?
A calf strain is an injury to the muscle fibres in the calf, either the gastrocnemius (the larger, visible muscle) or the soleus (the deeper muscle beneath it). Strains vary in severity, from a small number of fibres being overstretched to a more significant partial tear.

Calf strains in runners tend to develop in one of two ways: either a sudden acute injury from a sprint or jump, or a gradual overload injury from progressive training. This patient's injury was that her calf had been accumulating more load than it could handle, and eventually, the tissue couldn't keep up.
How We Assessed the Injury

When she came in to see our sports physiotherapist in Palmerston North, we started with a thorough assessment:
  • Ankle movement testing — to check range of motion and identify where movement was restricted
  • Functional load testing — we asked her to perform a double heel raise (rising up onto her toes). This immediately reproduced her pain, pointing directly to the calf as the source of the issue and indicating that it was currently aggravated by a very low level of load
  • Squat assessment — her squat was pain-free, helping us rule out involvement from the knee or hip
Based on the assessment, we confirmed a calf strain. The muscle was very tight following the injury, and this tightness was visibly affecting how she walked, causing her to compensate with every step.
The Treatment Plan
Calf strain recovery happens in stages. Rushing any of them increases the risk of reinjury, so the plan was progressive and guided by how her pain and function were improving.
Phase 1: Settle the Pain and Release the Muscle. In the first phase, the focus was on reducing pain and allowing the strained tissue to begin healing. We kept her completely away from running and skipping during this time. Treatment included:
  • Hands-on soft tissue massage — targeted massage to release the tight calf tissue and improve circulation to the area
  • Stretching exercises — gentle, pain-free stretches to begin restoring length to the muscle
Phase 2: Progressive Strengthening. As her pain reduced and her movement improved, we introduced strengthening work. This phase is critical — one of the most common reasons runners get reinjured is returning to training before the calf is strong enough to handle the load again.
Her strengthening programme included:
  • Calf raises off a step — lowering the heel slowly below the step edge before rising back up. This builds the strength throughout a larger range of movement, training the calf muscle to be strong at its full length, which is exactly what the calf needs for running
  • Ongoing soft tissue release — continued massage to maintain tissue quality as the muscle got stronger
Phase 3: Return to Running. When she was ready to return to running, we gave her a specific approach to protect her calf and reduce the risk of reinjury. The key principle: slow down before you build up.
When you run faster, your calf has to generate more explosive push-off force with every stride. By starting at a slower, more sustainable pace, you significantly reduce the demand on the calf while still allowing you to train. As an added benefit, starting slow also builds your aerobic base, rather than going out hard, getting breathless, and stopping early; a comfortable pace means you can sustain your run and build your capacity properly.

We guided her to:
  • Start running at a reduced, pain-free pace
  • Gradually increase duration before increasing speed
  • Continue her calf strengthening exercises alongside her return to running

The Result
With the right treatment and a sensible return-to-run plan, she made a full recovery and returned to running and skipping pain-free. Importantly, she came out of the process with stronger calf muscles and a clear plan for building her running safely, so she wasn't just back to where she was before; she was better equipped to keep progressing without breaking down again.

Why Runners Get Outer Hip Pain
The muscles on the outer side of your hip, are your single-leg stabilisers. Every time your foot hits the ground during a run, you're landing on one leg. In that split second, your outer hip muscles fire to stop your pelvis from dropping to the side and keep your whole lower limb aligned. That's significant work, repeated thousands of times per run. When you're a new runner, increasing your mileage, or pushing your pace, those muscles have to work harder than they're used to. How the Injury Presented
Our patient had been building up her running and starting to increase her pace when the outer hip pain began. It was the classic presentation: pain on the outer hip that increased with running, noticeably worse when running faster or further than usual.

How We Assessed the Injury
Outer hip pain in runners can come from a few different structures, so a careful assessment is important before starting treatment. We looked at:
  • Footwear — the shoes she was running in. Highly cushioned or unstable shoes can affect how load travels through the lower limb and increase demand on the hip stabilisers.
  • Single-leg stability — this is the key test for this type of injury. We asked her to stand on one leg, perform lunges, and do single-leg impact work like hopping. All of these reproduced her hip pain, confirming that the outer hip stabilisers were being overloaded.
The single-leg stability tests are particularly telling because they replicate what the hip is doing during every stride of a run. If you can't stabilise on one leg without pain, those muscles aren't ready for the repeated loading of running.

The Treatment Plan
Phase 1: Release the Overloaded Muscle. With the outer hip muscles in an overloaded state, the first priority was to reduce tension and calm the irritated tissue. We used:
  • Soft tissue massage — hands-on release work through the outer hip and gluteal muscles to reduce tightness and improve tissue quality
  • Dry needling — targeted needling through the hip muscles to address areas of tension and promote healing in the overloaded tissue
Phase 2: Strengthen the Hip Stabilisers. Releasing tight muscles is part of the answer, but it won't solve the underlying problem. The hip stabilisers need to get stronger, strong enough to handle the load of running without overloading. This is the most important part of the rehabilitation.
Her strengthening programme included:
  • Standing Clamshells — standing on one leg with the other knee bent and foot on the wall and opening the knee against resistance to isolate and strengthen the glute medius, one of the primary hip stabilisers, while also having to stabilize the pelvis
  • Side planks — building endurance in the lateral hip muscles in a position that challenges the whole stabiliser chain
  • Step-downs — standing on a step on one leg and slowly tapping the other foot down to the floor. This is essentially a single-leg squat and is one of the most effective exercises for developing the stability needed for running.
  • Hip hitching exercises — targeting the muscles that control pelvic movement and reinforcing control through the hip
This combination progressively loaded the hip stabilisers in ways that directly carry over to running, so that by the time she returned to full training, her muscles were ready for it.
Phase 3: Modify Training and Return to Running. One of the most reassuring things about this type of injury is that, in most cases, you don't have to stop running completely. Smart adjustments allow you to stay active while the hip gets stronger.
We advised her to:
  • Move to softer surfaces — shifting temporarily from pavement to grass or trail reduces impact load through the hip with every stride
  • Reduce her pace — running faster increases the stabilisation demand on the hip. A slower pace meant she could keep training without aggravating the injury.
  • Reduce her distance — shorter runs while the hip was still building strength, with a clear plan to build distance back up before increasing pace.
This modified approach meant she could stay active and maintain her fitness throughout recovery, which is far better for both recovery and motivation than stopping cold.

The Result
With consistent strengthening and modified training, her outer hip pain resolved fully, and she returned to running at her target pace. She also understood what had caused the injury and had the tools to prevent it from happening again as she continued to build her running.
